Scheduling your First Visit

The Process

Our team member will gather some information from you to help make that first visit as efficient as possible:

1. Name and date of birth of the patient.
2. Primary reason for seeking an orthodontic evaluation.
3. Name of primary dentist and date of last visit. We will call your dentist to have them send over any recent radiographs. If you can't remember the exact date of your last visit, or if you are seeking a new dental home, don't worry! We can still do an orthodontic eval.
4. Name of your dental insurance provider (if applicable) and related details so we can verify any orthodontic benefits prior to your consultation.
5. We love to thank our friends and patients for sharing our name - so please let us know how you heard about us!
